Living My Life

“Living My Life” is a disability support service designed for people whose injuries have resulted in long-term disabilities, impacting their ability to take part in everyday activities.

At Care On Call, we are proud to deliver all four components of the Living My Life (LML) program across Aotearoa New Zealand.

LML empowers kiritaki to take the lead in decisions about their care and support, aligning with the Enabling Good Lives principles. This person-centered approach emphasises increased choice and control, enabling individuals and their whānau to build meaningful, fulfilling lives.

Our Philosophy / Kaupapa

We believe in the transformative power of choice and control. By prioritising what matters most to each individual, we take a strengths-based approach to goal setting. This approach empowers kiritaki to connect with their community, live fulfilling lives on their own terms, and define and achieve goals that hold personal meaning.

Facilitated Pathway Mapping

Pathway mapping supports kiritaki and their whānau or support networks to identify and articulate their goals. We:

  • Create a safe, welcoming space for meaningful kōrero (conversations).
  • Build rapport to understand what truly matters to each individual.
  • Adapt to unique journeys, as no two pathways are the same.

Discussions evolve as deeper insights emerge, helping kiritaki visualise and refine their aspirations.

Independent Facilitation

We help kiritaki turn their goals into actionable plans by:

  • Exploring both free and paid community resources.
  • Supporting natural networks or agency staff, based on preferences.
  • Clarifying roles and funding responsibilities, including what ACC covers.

Our team ensures these plans are tailored, clear, and easy to understand, using aids to support communication when needed.

Coaching to Self-Manage

We empower kiritaki to take control of their care by:

  • Helping them decide what they can manage independently and what they’d prefer to delegate.
  • Providing guidance on employment and HR practices when hiring personal support staff.
  • Simplifying ACC self-management processes so any decisions are informed and confident.

Tailored Supports

We provide tailored support to help kiritaki achieve their goals, focusing on:

  • Strengthening social connections, hapori involvement, and meaningful mahi (work).
  • Building essential living skills for greater independence.
  • Delivering flexible, personalised support to meet individual needs.

This is made possible by our skilled team of Support Workers and the strong, collaborative relationships we’ve built within the wider community.
